If you are Algar telecom user, then you can login to the official web page of Algar and see your PUK code in account management page.

Vivo Users : From another Vivo mobile number, call to the customer care number 8486 or 1058
If you are calling Vivo customer care from other network or land line then call 0800 772 8346
Sercomtel Users : From another Sercomtel mobile number, call to the customer care number 1051
If you are calling Vivo customer care from other network or land line then call 0800 400 0430
Oi Rio Users : Rio de Janeiro Metropolitan Area. (21) 4004 0001. Other regions throughout Brazil. 0800 729 0722.
The customer care person will ask the SIM number printed on your SIM card which contains 8 to 15 numbers.
You have to tell the owner of the SIM card which is registered on the name while buying it.
Some other basic details to Confirm that you are the owner. If you manage to convince the customer care person, then they will provide the PUK code.
